Transfer Admission
If you are a transfer student, the following steps will be required for admission to the program:
- Apply online to Stanislaus State (Cal State Apply). Please note: $70 application fee is required.
- Submit Official transcripts from each college attended (including in-progress), must be sent electronically to CAPEtranscripts@csustan.edu. If your college does not have an electronic transcript option, please mail your transcript to:

Current Stanislaus State Student
If you are a current Stanislaus State student, the following steps are required to change your major to Health Science:
- Complete the Pre-Admission Form form
- Email the completed form to the Program Director, Mechelle Perea Ryan, mperearyan@csustan.edu
- Once you receive confirmation that you meet all program entry requirements, you must then complete the change of degree form
